Reserve Book List Form

The following form is for the use of faculty and instructors wishing to submit requests via the web for books to be put on reserve. This form may be used for requests to either the McHenry or Science & Engineering Libraries. This form is for library book requests only.
For photocopy submissions please bring those to the appropriate library's reserve desk. Photocopies or personal material must be on 8.5 x 11 paper and you must cite author, title, and original source for each article or photocopied chapter from a book.
