ERes Course Page Request Form Information

Dear Instructor:

Before filling out the on-line ERes Course Page Request form please take a minute to read the following instructions.

When filling out this form there are a few things to remember.

  1. Your course page will be administered by the Reserves unit (McHenry or Science & Engineering) handling your reserve book list. Please select the correct unit.

  2. You must include a password. All course pages in ERes must be password protected. As the instructor, you are responsible for giving the password to the students in your class.

  3. You must have a copyright compliance form on file for your course this quarter before your ERes course page can be processed. If you have already signed one for your reserve book list, you don't need to sign another for that particular course. A copy of this form is available on-line or at either library's Reserves unit.

  4. Course Page requests are processed in the order they are received.

For your convenience: After filling out the course page form there is a link that will take you to a .pdf version of the Copyright Compliance form. You may print it out, sign it, and mail it to the appropriate Reserves unit.
