If you are a Courtesy Patron or a Concurrent student.
If you are having difficulties using Melvyl or Citation Linker.

Use these forms to request Interlibrary Loan/Document Delivery items, books from other libraries, or books held at the Northern Regional Library Facility (NRLF). Courtesy borrowers, concurrent enrollment borrowers, UCSC faculty, staff and students may all use these forms.
Note: Use of this form will increase the time required to process your request. For faster service, UCSC faculty, staff and students should use CDL Request, if possible.
